Is Your Leg Pain Really Coming From Your Spine — Or Something Else?

If you have pain travelling down your leg, you’ve likely been told it’s Sciatica — and that it must be coming from your spine.

But that’s not always true.

According to consultant spinal neurosurgeon Anthony Ghosh of The Spine MDT, while spinal nerve compression is a common cause of leg pain, it’s far from the only one.

In fact, MRI scans often show disc bulges and degeneration in people who have no pain at all — meaning not everything seen on a scan is actually the cause of symptoms.

When Leg Pain Is Coming From the Spine

Leg pain can originate from the spine when a nerve is irritated or compressed.

Common spinal causes include:

  • A slipped (herniated) disc pressing on a nerve
  • Foraminal stenosis (narrowing where nerves exit the spine)
  • Central spinal stenosis (narrowing of the spinal canal, often painful when walking)
  • Less common causes such as cysts or tumours

Typical signs of spinal nerve pain:

  • Pain starting in the lower back or buttock
  • Pain travelling down the back of the thigh into the calf or foot
  • Numbness, tingling, or weakness

When Leg Pain Is Not Coming From the Spine

Several non-spinal conditions can closely mimic sciatica — and are often misdiagnosed.

Common alternatives include:

  • Hip problems (e.g. arthritis or cartilage tears)
    → Often cause groin pain or discomfort when crossing legs
  • Gluteal tendinopathy
    → Strain or small tears in the buttock tendons
  • Piriformis syndrome
    → A tight hip muscle irritating the sciatic nerve, often worse when sitting
  • Sacroiliac joint dysfunction
    → Pain near the base of the spine, often worse in the morning
  • Vascular claudication
    → Leg pain that builds when walking and eases with rest
  • Peripheral nerve injuries
    → Nerve damage near the knee affecting the lower leg or foot

👉 These conditions can feel remarkably similar to sciatica, which is why careful diagnosis is essential.

Why MRI Scans Don’t Always Tell the Full Story

One of the biggest misconceptions is that imaging gives a definitive answer.

In reality:

  • Many people have disc bulges without pain
  • Age-related changes are often normal findings
  • The “abnormality” on a scan may not match your symptoms

👉 This is why relying on imaging alone can lead to the wrong treatment.

Why Accurate Diagnosis Matters

The most important takeaway is this:

Treatment should be guided by symptoms and clinical assessment — not just scan results.

If your leg pain:

  • Isn’t improving with spinal treatment
  • Doesn’t follow a typical nerve pattern
  • Or keeps coming back

...it may be time to reassess the diagnosis.

Identifying the true source of pain is far more effective than chasing MRI findings.
